The 1779 Packet
Often called the One Page Declaration. The declarant uses the version that matches his or her circumstances and supplies the identity evidence below.
Core Packet Checklist
- Applicable 1779 Declaration Use the form matching birthplace and employment status.
- Witness Testimony Affirmation No. 1 Completed by the first qualifying witness.
- Witness Testimony Affirmation No. 2 Completed independently by the second qualifying witness.
- Identity Record Color copy of a birth or naturalization certificate.
